Description Carotic endarterectomy belongs to the most often procedures in vascular surgery. The authors present a case of a patient with extreme arteriosclerosis of cerebral arteries, who underwent successful surgery in the cervical plexus block. The patient with the complete closure of right cerebral arteries was scheduled for desobliteration surgery of the left internal carotic artery with 95% stenosis. This case demonstrates the effectiveness and safety of regional anaesthesia in a patient with high anaesthesiological risk.
