Determination of accuracy of the transit time with respect to defecography in the diagnosis of anorectal dysfunction: use in practice

Popis | Aim: Diagnosis of anorectal dysfunction requires a comprehensive diagnostic approach. The aim of this study was to determine the accuracy of transit time (TT) considering the method defecography and to find out what result of TT is able to predict a significant finding in defecography. Methods and materials: From January 2006 to September 2009 we examined 100 patients with constipation. In all these patients defecography and TT were performed subsequently. Patients were divided according to the result of transit time and defecographic finding into 2 groups. The first group included patients with any marks stasis in the colon (TT-1 and TT-2) and with significant finding in defecography (D-2 and D-3). The second group of patients had marks stasis only in left colon according to TT (TT-2) and also an significant finding in defecography. The results were processed statistically using ROC analysis and the analysis of contingency tables testing using Chi-square test. Results: Based on the statistical results we have found that in group I. the sensitivity of the TT method was 83.3% and a specificity 48.1% ; in the second group 72.9% and 84.6%. In the case of a significant finding in defecography (pathology of 2nd or 3rd grade) the overall accuracy of transit time method (AUC) was determined to 78.2%. Conclusion: We were able to demonstrate that TT can predict a important finding on defecography, while in the 2nd group of patients specificity has increased significantly, 84.6%. The overall accuracy of the TT method (AUC) was set at 78.2%. |
