Intracranial pressure - monitoring during stereotactic evacuation of infratentorial hematomas

Popis | The autors presents a series of 14 patients with spontaneous intracerebellar hematomas evacuated by frame based stereotactic technique, in 4 patients we have measured intracranial pressure (ICP) during stereotactic evacuation. This method was indicated in patientswith stabil neurological status according ro Glasgow Coma Scale (GCS) in patients with GCS more than 8. The period between hemorhagy and operation was less than 24 hours in 14% patients, the rest of them was operated in period from 24 to 72 hours. In this study 85% od patients had initial decompensated hypertension and 43% of patients had initial coagulopathy. We have used frame based stereotaxy with Riechert-Mundinger apparatus with CT localisation of target and optimal trejectory. Out of 14 patient cosidered, the immediate postoperative improvement of the neurological status was found in 64%, no change in 29% and deterioration in 7% od patients. The relationship between the peroperative ICP and the size of hematoma on CT scan and the time of operation was studied to evaluate their influence on the therapeutic results. |
